Is Bedford Care Right For You?


The challenge of choosing care outside your home for yourself or a loved one is a very difficult task. Many are overwhelmed with knowing where to start. We would like to offer a few tips to assist you in making the best decision for you or your loved one.

  • If making the decision for a loved one, do your best to involve them in the decision making process as much as possible.
  • Listen and validate their feelings of fear, concern, anxiety and uncertainty about their future.
  • Learn as much as you can about the potential residents medical condition(s).
  • Take time for yourself as a caregiver. Neglecting your own needs can lead to stress, burnout and feelings of resentment.
  • Accept support from others.

Admissions Checklist:


Prior to admission to a Bedford Care Center the facility will require certain documentation related to the future resident’s health and needed medical care.  Your healthcare provider can assist you in obtaining the following information needed for admission.
  • Physician’s orders, including a diet order, signed by the physician with a diagnosis for each ordered medication
  • Medical History and Physical Exam dated within 30 days prior to the date of admission
  • Current Chest X-Ray dated within 30 days prior to the date of admission
  • TB Signs and Symptoms form completed and signed by a physician or nurse practitioner within 30 days prior to admission
  • TB Skin Test with Step 1 results documented within 30 days prior to admission
  • A photo ID and copies of all current insurance cards
  • Power of Attorney, Conservatorship, or other legal documents
  • Healthcare Advance Directives such as a Living Will

If admitted from a hospital, the following additional information is needed:
  • Discharge Summary
  • Physicians Certification
  • Medication administration records for past 7 days
